In order to find out if you are eligible for home health care services you need to consider the following:
- Are you under a physician’s care? Did the physician authorize the services?
- Usually a homebound criterion is required for home health services.
- Do you (patient) require any of the following: a speech therapist, a physical therapist, or a speech therapist?
- You must have seen a doctor in the last 60 days.
- You must have a primary care physician.
Qualifying for Home Health Care Service
To qualify for services you must meet these requirements:
- You need assistance part of the time, or occasionally.
- You must have a doctor, and that doctor must oversee your care.
- Your doctor must certify that you cannot leave your home with a lot of help.
- Your doctor must authorize the services.
- You need services that can only be given by a nurse or therapist.
